Please complete a referral form through UHN’s Epic system by using the “Transcribe Order” function. Under department, please select “UC-Schroeder Pain Assessment and Rehabilitation Research Centre”. Under site, please select “University Centre”. Under service, please select “Schroeder Pain Assessment and Rehabilitation Research Centre”, and follow the prompts.
